Before filling out your application, OCOD Club Members should email [email protected] to obtain your work credit discount coupon code.

Before filling out the application, please take a photo of your current vaccine record in order to have it ready to upload.

Be sure to fill out all of the STARRED fields (most are starred.)

After submitting your Application, you will be sent to a new page  to schedule and pay for your class.

If you filled out your correct email address, you will receive an emailed receipt.  Please add [email protected] to your contact list to avoid having it go to your SPAM.
